HOSPITAL AFTER CARE IN FOD
Age Concern Forest of Dean FREE HOSPITAL AFTERCARE SERVICE IN THE FOREST OF DEAN WHO QUALIFIES: Anyone over 55 who has recently been discharged from hospital and requires short term assistance. WHAT DOES THE SERVICE INCLUDE? - Short term social and moral support
- Weekly visit from a Volunteer for a minimum of one hour for 4 to 6 weeks
- Help with daily tasks such as:-
Light shopping
Answering letters
Paying bills
Chatting over a coffee
Going for a walk
(This does not include personal or domestic care)
HOW IS THE SERVICE ACCESSED:- If you have recently been discharged or are about to be discharged from hospital, or you know of anyone in this position who would benefit from this service, please call us on: 01452 422660 9 am - 4pm Monday to Thursday 9 am - 3.30 pm. Friday
